How specifically depends on the expression, but generally simplification means that you have as few exponents, brackets and fractions as possible. The resulting expression should visually look simpler than the starting expression.

It depends on the expression in question but normally it will require multiple steps of collecting like terms, expanding brackets and balancing equations.
